MINNEAPOLIS – Visiting Nurse Services of Connecticut, one of the state's oldest and largest home health agencies, announces that it has entered into a business relationship with Cardiocom to provide advanced home telehealth technology to patients with chronic conditions using Cardiocom's new Commander FLEX™ device.  Cardiocom®, Experts in Telehealth(SM), is a world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.

Cardiocom's telehealth system provides an interactive opportunity to proactively manage patient care.  Every day, VNS ofConnecticut patients use the Cardiocom Telehealth System to communicate with a VNS of Connecticut nurse.  The Commander FLEX guides the patient through an interactive Health Check and collection of vital signs including: weight, blood pressure, oxygen saturation and heart rate.  This information is immediately transmitted via a standard phone or a cellular signal.

VNS of Connecticut is one of the state's oldest, largest and most comprehensive providers of skilled home health and hospice services, having emerged from its beginnings as the Bridgeport VNA to rescue at risk home care services in Fairfield, New Haven and Litchfield counties.  The agency currently serves more than 7,000 patients each year, making more than 250,000 home care visits.  A national 2010 HomeCare Elite agency, VNS of Connecticut utilizes disease management programs, protocols and telehealth technologies to focus on heart failure, COPD, hypertension and diabetes to provide services for the chronically ill in their homes and to prevent hospital re-admissions.  The agency's readmission rates are traditionally among the lowest in the state.  Also, VNS' Hospice at Home™ program, created in the mid 1980's, was one of the nation's first Medicare certified end-of-life programs.  Agency staff  have since augmented the hospice program to offer palliative care treatments for patients likely to enter hospice service.
