my1HIE(sm) Implements WellCentive Registry

ATLANTA and BINGHAM FARMS, Mich. – WellCentive, LLC today announced that WellCentive Registry has been selected by my1HIE (My One Health Information Exchange) as its patient registry and disease management system.

Through the use of WellCentive Registry, more than 1,000 my1HIE physicians and other health care providers in Southeast Michigan will be better able to provide preventive care services and track and manage patients with chronic health conditions, such as diabetes, asthma or heart disease. WellCentive Registry combines the capabilities of a Web-based, community-wide, point-of-care all-patient registry system with custom interfacing, customizable alerts, targeted patient outreach, comprehensive reporting tools, secure messaging features and direct reporting of outcomes to a variety of payors. WellCentive Registry includes highly effective patient outreach tools, including an integrated automated telephone appointment and care reminder system called WellCentive Outreach.

my1HIE is a unique, physician-driven health information exchange launched in November 2008. With the physician and their respective physician organization at the forefront of the complete health information exchange, four physician organizations representing more than 3,000 physicians are already committed to my1HIE, and there are discussions in the works with at least four additional physician organizations representing an additional 2,000 physicians.
