DALLAS – The 250-physician Banner Arizona Medical Group (BAMG) has selected Phytel, the industry leader in automated, physician-led health improvement, to help the group develop its population health management program and improve patient outcomes. BAMG, which is part of Banner Health, a not-for-profit healthcare system with 23 hospitals in seven states, plans to roll out Phytel’s Proactive Patient Outreach and appointment reminder services to primary care doctors and specialists in all of its sites.

“Phytel will be very helpful to us in standardizing our quality improvement program and ensuring that all of our patients’ preventive and chronic care needs are met,” said Jim Brannon, CEO of Banner Arizona Medical Group. “We established the BAMG partnership with Phytel as part of our strategy to manage population health and improve the coordination of care.”

Using data drawn from practice management systems and electronic health records, Phytel builds population-wide registries showing the health status and recommended care of all the patients in a practice. Evidence-based protocols, customizable by each group, trigger communications to patients to contact their physician when they need preventive or chronic care services.

“We welcome Banner Arizona Medical Group to our rapidly growing list of customers,” commented Steve Schelhammer, CEO of Phytel. “We look forward to working closely with Banner Health to help its physicians improve quality while enhancing the patient experience.”
