BERGEN COUNTY, NJ – Englewood Hospital Home Health and Hospice Services has agreed to a partnership in order to provide chronically ill patients with the option of telehealth services.

Englewood Hospital, a Medicare-certified home health agency, has entered into an agreement with Cardiocom, a manufacturer of telehealth solutions in the areas of heart failure, diabetes, hypertension and other chronic symptoms. Englewood will be using Cardiocom Commander Telehealth System in an attempt to be more connected to patients outside of the facility.

"By transmitting objective, real-time health data every day, the patient is connected to our medical team so that their care plan can be adjusted as needed," says Katherine Clark, a nurse and  manager of the telemedicine department. "This system uses today's most advanced home care technology and monitoring services."

The interactive telehealth system now available to the hospital communicates patients' vitals to an agency where the data is monitored by an Englewood Hospital Home Health nurse. The patient is brought through the system in a step-by-step process that collects vital health information such as weight, blood pressure and heart rate.

The compiled information is sent through an active phone line to the patients care provider for a review.

"We anticipate that Cardiocom's unique system will improve patient engagement and quality of life," adds JoAnn Walter, a nurse and  executive director of continuing care.
