WARREN, MI – St. John HealthPartners, a physician-hospital organization, is rolling-out Web-based clinical support to help manage patient populations and screen for chronic diseases.
SJHP has signed a deal with Ann Arbor, Mich.-based Cielo MedSolutions to acquire licenses for Cielo Clinic software.
St. John HealthPartners is comprised of St. John Health and more than 2,200 physicians on its local medical staffs. St. John Health, a ministry of Ascension Health, is comprised of seven hospitals and more than 125 medical facilities in southeast Michigan.
Under the agreement, SJHP acquires Cielo Clinic software not only for the primary care clinics operated by St. John Health, but for the network of affiliated, private medical practices also included in St. John HealthPartners.
"We began adopting Cielo Clinic in 2007, and have now rolled out the software to 23 sites with more than 250 clinicians," said William Isenstein, executive director of St. John HealthPartners. "It's become clear that all of our primary care providers, ranging from family medicine to obstetrics/gynecology, internal medicine and pediatrics, can greatly benefit from incorporating Cielo Clinic into their clinical operations."
Cielo Clinic is a clinical quality management system designed to proactively manage the care needs of medical practice and health system patients through a patient registry, a clinical decision support engine and population management functionality.
"Cielo Clinic is a tool to reach the Holy Grail for a primary care physician: proactive preventive care," said Cherolee Trembath, MD, chairperson of family medicine at Providence Hospital in Southfield, Mich. "I can go to one source to find all my patients with a particular condition, rather than counting by hand or going in and out of many different databases."
