FOSTER CITY, CA – The California Association of Physicians Groups (CAPG) has enlisted Health Access Solutions to help with its accountable care efforts – bringing the company's web-based multidisciplinary care coordination platform to more than 150 physician groups and nearly 15 million patients.
Recent healthcare reform legislation, such as the Patient Protection and Affordable Care Act (PPACA), highlights the critical need for a patient-centric, multidisciplinary delivery system to coordinate care while balancing quality, utilization of services and cost.
Patient-centered medical home (PCMH) and accountable care organization (ACO) models are outlined within PPACA as effective methods to achieve this degree of collaboration – as is the need to develop effective healthcare IT to support these forward-looking strategies.
Executives from CAPG – which is co-producing the National ACO Congress that's being held through Oct. 27 in Los Angeles – say the organization is committed to improving healthcare in California (and nationwide) by way of the ACO-style care environment.
