Chronic disease managementis an approach to coordinated healthcare interventions and communications for populations with long-term conditions, in which patient self-care is significant.
Disease management supports the physician/patient relationship and plan of care. It emphasizes prevention of exacerbations and complications using evidence-based practice guidelines and patientempowerment strategies. Disease management also evaluates clinical, humanistic, and economic outcomes on an ongoing basis with the goal of improving overall health.
